For all coaches/managers,

We now have an official "check" request form that is linked on your team homepages at the bottom (Go to the "Teams" pulldown from the top menu and select your individual team to reach it's homepage).  This is only way we will accept check requests going forward.

Make sure you are logged into your account on sportsengine and then fill out the request.  It will be sent automatically to the treasurer for review and disbursement.  Please be descriptive on what the purpose of the check is and make sure the activity date is correct (i.e. date of the game you are paying for or first day of tournament). 

If the request is for multiple teams, make sure you list the cost breakdown for each team in the purpose box.
